THE CLINICAL PHARMACOLOGY OF ANTIEPILEPTIC DRUGS

Research activity

Over the past 30 years, our main research activities have focused on issues related to the pharmacological treatment of epilepsy. Issues addressed in these studies include the pharmacokinetics of antiepileptic drugs in healthy volunteers and in patients with epilepsy, pharmacodynamic characterization of specific drug effects, drug utilization and drug surveillance, and outcome assessment through clinical trials, including innovative study designs. Many of these studies have involved extensive collaboration with clinical centres in Italy and other countries in Europe and the rest of the world.
Specific areas covered by our research interest include the following:
1. Randomized controlled comparative trials, including placebo-controlled multicentre double-blind trials, assessing the efficacy and tolerability of old and new generation antiepileptic drugs in people with epilepsy;
2. Studies on factors influencing quality of life (QoL) in people with epilepsy, with special reference to the characterization of adverse drug effects as determinants of QoL;
3. Development and testing of innovative paradigms to demonstrate the efficacy of antiepileptic drugs;
4. Studies on national and international differences in the utilization of antiepileptic drugs;
5. Investigation of drug-drug interactions influencing response to treatments in patients with epilepsy;
6. Studies on the consequences associated with antiepileptic drug treatment during pregnancy and lactation, including a large international pregnancy registry to compare prospectively rates of major congenital malformations after prenatal antiepileptic drug exposure. The registry has already enrolled over 15,000 pregnancies.
7. Characterization of the factors affecting intra- and interindividual variability in the pharmacokinetics of antiepileptic drugs, including the influence of mode of administration, age, pregnancy and co-morbidities;
8. Development and validation of assays for the determination of drugs in body fluids.

Comparative strenghts of the research unit in a 3-5 year time frame


The research unit has strengthened collaboration with a network of centres of excellence in Europe and the U.S. to facilitate the conduction of clinical drug trials in patients with epileptic disorders, including pediatric populations. This should facilitate access to EU funding and further opportunities for collaboration with industry.
